(1) The registration certificate and business license of imported medical equipment issued by the State Food and Drug Administration;
(2) For medical equipment containing pharmaceutical ingredients, China mandatory certification (special goods approval form) should be provided;
(3) Automatic import licence (O Licence) is required for some equipment;
(4) Equipment picture, nameplate, equipment technical parameters, end use, Chinese manual, etc.;
(5) Import contract, packing list, invoice;
(6) Other information that needs to be added。
2.Precautions for customs inspection of imported medical equipment:
(1).The choice of port of entry ---- is determined according to the quantity of goods and familiarity.
(2) Whether the customs declaration is complete - in addition to importing general goods, there are also documents required for importing medical equipment, which should be prepared one by one。
3.The receiving unit of imported medical equipment shall have the following qualifications:
(1) Medical equipment business license;
(2) Business license (business scope with medical equipment sales license);
(3) Import and export rights (if there is no qualified import and export company can import)
(4) Medical device registration form。
